ORDER FORM: Print out form, complete and mail with photos, artwork & deposit

What we need from you:

Supply us with your photograph(s), use a minimum of a 4 x 6 and a maximum of 8 x 10, artwork for logos (camera ready), desired text for front and back as well as any other information you feel may be useful.

Terms and Conditions:

	The customer guarantees the legal propriety of all material submitted, and insures all material submitted does not violate copyright or trade names.
	Orders are shipped approx. 10 to 15 business days from proofing as space permits
	75% deposit of total amount is required with order, balance C.O.D.
	Shipping charges from Cobourg, Ontario are extra if outside local area - UPS standard unless otherwise specified (tracking number provided once shipped)
	Applicable taxes - 7% GST on all orders inside Canada; 8% PST for Ontario residents or supply completed PST exemption form with order
	Where the customers instructions are lacking or incomplete, PostCards Plus may at our own discretion, use type style and composition as it deems to be in the best interest of the customer.
	Quantities are not exact 95% to 115% of order will constitute a full order
